HealthSnap Secures $25M to Scale AI Virtual Care
The Miami-based virtual care company, HealthSnap has secured a $25 million senior secured growth financing facility led by Eastward Capital Partners. The financing will refinance existing debt, strengthen the balance sheet and provide additional capital for AI development, commercial expansion and wider deployment of HealthSnap’s Advanced Primary Care Management platform.
HealthSnap combines Remote Patient Monitoring, Chronic Care Management, Principal Care Management and Advanced Primary Care Management with AI-supported clinical workflows, analytics, reimbursement tools and care coordination in one EMR-integrated platform.
The company says it now supports more than 80,000 active patient programs across 200 health systems and physician organizations, with more than 100,000 programs projected by the end of 2026. Its partners include Prisma Health, AdventHealth, Ascension Health, Sentara Health, Tampa General Hospital, UnityPoint Health and others. Several healthcare organizations, including Sentara, Tampa General and UnityPoint, have also invested strategically in the company.
AI Is Moving Deeper Into the Care Workflow
Earlier this year, HealthSnap launched an AI-augmented Advanced Primary Care Management solution that combines agentic AI with clinical workflows to help care teams manage larger patient populations.
Its AI is already being applied to progress-note review, clinical summaries, nurse training and routine patient interactions such as enrollment, device troubleshooting and compliance outreach. HealthSnap says AI reviewed more than 3 million progress notes in 2025. Clinical recommendations remain subject to clinician-defined protocols and safety guardrails, with physicians and care teams retaining final judgment.
That human oversight is important. HealthSnap’s model is built around using AI to increase clinical capacity rather than remove clinicians from the workflow.
The company is also reporting stronger business momentum. Revenue has increased more than fivefold over the past three years, including 40% year-over-year revenue growth and a 47% year-over-year improvement in EBITDA. These figures are company-reported.
Clinical studies cited by HealthSnap also associate its remote monitoring programs with improvements in blood-pressure control and lower healthcare utilization, although those findings relate to specific patient cohorts and should not be generalized to every deployment.
