Hopscotch Raises $53M to Scale Rural Primary Care Across America

Hopscotch Primary Care is building its healthcare model around a simple idea that becomes much harder to deliver outside major cities: older adults should not have to trade where they live for reliable access to a doctor. Founded in 2021, the company now serves more than 15,000 patients across 12 clinics in the Southeast, concentrating on rural communities where access to physicians and coordinated care can be limited.

The company has raised $53 million in Series D funding, led by 8VC and Town Hall Ventures. New backers include the Autism Impact Fund, John Doerr, Dr. Richard Merkin, and the Leon Levine Foundation, while aMoon Fund, Citi Impact Fund, Alumni Ventures, and K2 HealthVentures returned. The capital will help Hopscotch enter additional rural markets and expand the technology and operations behind its care model.

Hopscotch is led by CEO Tim Gronniger, whose background spans value-based care, healthcare policy, and leadership roles involving CMS and the White House.

Its approach gives Medicare patients more than a conventional clinic visit. Hopscotch offers same-day appointments, longer consultations, multidisciplinary care teams, 24/7 access, proactive outreach, and support beyond the clinic. Technology works behind those relationships, helping care teams manage information and administrative work while providers spend more time focused on patients.

The more interesting test is whether that experience can remain intact as Hopscotch grows. The company reports an 89 Net Promoter Score, retention above 90% and more than 25 percentage points of medical-loss-ratio improvement during patients’ first two years. It also says its Western North Carolina operation is already profitable.
